- You get the opportunity to build authentic relationships with the individuals you support.
- You assist the agency nurse in educating, training and coaching direct support staff to follow each individual’s health care plan.
- You work with other community healthcare providers to care for individuals of all ages with intellectual and developmental disabilities.
- You provide phone triage, including a shared after hours on-call rotation.
Minimum Job Requirements
- Current LPN Certification
- A valid driver’s license with no more than 5 points on driving record
- Experience supporting individuals who have developmental disabilities and their families.
- Previous nursing experience.
- Interest in supporting individuals with disabilities.
- Demonstrate reliability regarding basic work habits such as attendance and punctuality.
- The ability to work as a team member.
- Low physical effort required to sit, stand at files, bend, stoop, lift, and walk.
- Maximum unassisted lift = 50lbs.
- Local travel required.
- Primarily day shift M-F; occasional weekend/evening hours; on-call rotation
