Key Responsibilities:

  • Develop and maintain individualized care plans in collaboration with patients, and case managers.
  • Coordinate schedules for caregivers and ensure the timely delivery of home healthcare services.
  • Serve as the primary point of contact for caregivers and patients, addressing concerns and providing updates.
  • Monitor patient progress and update care plans as needed.
  • Ensure compliance with all state and federal healthcare regulations.
  • Maintain accurate records and documentation for all patient interactions..

Qualifications:

  • Bachelor's degree in Nursing, Social Work, or a related field (preferred).
  • Experience in healthcare coordination, case management, or a similar role.
  • Strong knowledge of home healthcare practices and regulations.
  • Exceptional organizational and time-management skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency in using healthcare software and technology.
  • CPR certification (preferred).

In this role, you will be the primary point of contact to ensure that patients recieve receive exceptional care. You will manage patient scheduling, maintain medical records, and assist with administrative tasks to ensure a smooth operation. Your ability to communicate effectively and handle sensitive information with discretion will be crucial in providing a positive patient experience.

Responsibilities

  • Greet patients warmly and assist them with the check-in process.
  • Schedule appointments efficiently while considering patient needs and provider availability.
  • Maintain accurate and up-to-date patient records, ensuring compliance with medical terminology standards.
  • Answer phone calls and respond to inquiries regarding services, appointments, and insurance information.
  • Collaborate with healthcare professionals to coordinate patient care and follow-up services.
  • Assist in managing billing processes and verifying insurance coverage for patients.
  • Ensure the office environment is welcoming, organized, and compliant with health regulations.
  • Provide administrative support as needed to enhance office efficiency.

Experience

  • Previous experience in Health Care Coordination is required.
  • Familiarity with medical terminology is essential for effective communication within the healthcare team.
  • Strong organizational skills and attention to detail are required to manage multiple tasks effectively.
  • Excellent interpersonal skills to interact compassionately with patients and their families.
