The Opportunity
Eons Care Management program offers a unique approach to patient engagement andadherence tracking. Using our EPM software, the Care Management teamincluding PatientAdvocacy Coordinators (PACs)works closely with patients and provider offices to facilitatecommunication and scheduling on behalf of our healthcare customers.
The PAC is responsible for managing inbound and outbound communications with patients andproviders, focusing on facilitating timely follow-up appointments and supporting adherence torecommended care pathways. This is a non-clinical, administrative role that supportsoperational workflows through structured communication, data entry, and coordination.
Your Role in Patient Advocacy
The PAC role focuses on communication, scheduling coordination, and data entry to supportcare pathway adherence. The PAC acts as a liaison between patients and provider offices toensure follow-up care is scheduled appropriately, but does not involve clinical decision-making,diagnosis, or treatment planning.
Key Responsibilities
- Conduct structured outbound calls to patients and providers, following standardized call scripts to remind and coordinate overdue or upcoming follow-up appointments.
- Answer inbound calls from patients and providers, addressing basic questions and concerns using standardized responses and protocols.
- Log and document all call interactions and outcomes within the Eon EPM platform to ensure accurate communication records.
- Escalate clinical or complex inquiries beyond scripted responses to appropriate healthcare providers or internal supervisors for resolution.
- Update patient records with follow-up information or scheduling updates as provided by patients or providers.
- Deactivate patients within the EPM system based on clearly defined deactivation criteria.
- Participate in quality assurance initiatives and support internal process improvement projects.
- Generate and deliver reports within Eon EPM as requested by internal teams.
