Purpose
Oversees between 5-8 Rehabilitation Specialists. Coordinator empowers Rehabilitation Specialists to provide rehabilitation services to assist assigned individuals in achieving an optimal quality of life including general dependency treatment (COPSD), employment related skills to increase consumers’ independence and ability to maintain community tenure. Coordinator works alongside the Director of Rehabilitation Services to provide training, coaching, and supervision to Rehabilitation Specialists. Team Lead seeks to help specialists achieve all goals set out by the Director.
Qualifications:
- Bachelor’s degree from an accredited college or university with a major in social, behavioral or human services related field: (Psychology, Social Work, Medicine, Nursing, Rehabilitation, Counseling, Sociology, Gerontology, Special Education, Physician Assistant, Educational Psychology, Human Growth and Development, Early Childhood Education, Early Childhood Intervention)
- Two (2) years paid direct care experience in a community setting with knowledge of MH rehabilitative techniques and community resources
- Bilingual (English and Spanish) preferred
- Valid Driver’s License with safe driving record and current automobile insurance
- Maintain the Adult Needs and Strengths Assessment (ANSA) and Child and Adolescent Needs and Strengths (CANS)
Essential Duties and Responsibilities:
- Develops personal growth opportunities for the Rehabilitation Specialists
- Coach and counsel Rehabilitation Specialists through planning, monitoring, and appraising job results
- Provide training for Rehabilitation Specialists focusing on implementing company policies and procedures
- Conduct monthly reviews of Rehabilitation Specialists cases to find ways to encourage proper reporting and improved consumer growth
- Oversee and monitor documentation produced by Rehabilitation Specialist (progress notes, reviews, authorizations, treatment plans) as needed
- Meets with Rehabilitation Specialists on a one-on-one basis to help staff improve the quality and content of documentation
· Ascertains that documentation that is returned with discrepancies is corrected on a timely manner (within 24 hours)
Rotate and provide monthly trainings to rehab specialist
· Conduct Quarterly Peer Reviews
· Assist Rehabilitation Specialist with visits when necessary
· For any changes on caseloads inform the necessary departments such as director, billing, data entry, and payroll
- Monitor staff hours for full time and part time daily
- Conduct mandatory monthly trainings to assigned Rehabilitation Specialist team
- Oversee case load of Rehabilitation Specialist
- Create Productivity report / Audit report for each individual Rehabilitation Specialist
- Maintain a case load of consumers if the department requires it
- Contribute to the team effort by accomplishing related results as needed
- Work with Rehabilitation Specialist to decrease Dr. appointment “no-shows”
- Coach and mentor Rehabilitation Specialist to increase their billable hours and meet La Familia Del Paso’s minimum billable services standard
- Decrease errors and notes returned for corrections
- Other Duties as Assigned

Pay: $41,600.00 - $45,760.00 per year
