Job Description
The Home Health Aide/Patient Care Technician provides personal care and related services in the patient home under the direction of licensed nursing staff and within the scope of defined practice. Demonstrates a commitment to accountability and growth to maintain and expand knowledge and skills. Serves as a liaison between other agency Associates ensuring communication and accurate records are available.
Essential Home Health Aide/PCT Job Duties 1. Follows the Home Health Aide/PCT Care Plan and/or assignment per the licensed nurse, the Home Health Aide/PCT will provide the following: a. Helps the patient to maintain good personal hygiene. b. Assists in maintaining a healthful, safe environment. c. Plans and prepares nutritious meals, markets when instructed to do so by the licensed nurse. d. Assists the patient with ambulation. e. Assists with certain treatments as ordered by the physician and approved and supervised by the nurse. f. Assists the therapy personnel as needed with rehabilitative processes. g. Encourage the patient to become as independent as possible according to the nursing care plan. h. Attempts to promote the patient’s mental alertness through involvement in activities of interest. i. Gives simple emotional and psychological support to the patient and other members of the household. j. Establishes a relationship with patient and family which transmit trust and confidentiality. k. Prepares a report of her visit on the day it is performed and incorporates same in the clinical record weekly. l. Reports any change in the patient’s mental or physical condition or in the home situation to his/her immediate supervisor or licensed nurse. m. Carries out his/her assignment as instructed by the nurse and/or reports to the licensed nurse if unable to do so. n. Works with personnel of other community agencies involved in the patient’s care as directed by the licensed nurse. o. Performs routine housekeeping tasks as related to a safe and conformable environment for the patient as instructed by the licensed nurse. p. Attends in-services as required by agency, state, and/or federal regulations.Position Type
