Outpatient Therapist/Assessor
The Outpatient Therapist/Assessor is a clinical position dedicated to providing behavioral health services in a manner that upholds the Epic Health Partners’ mission, quality standards, and results in better client behavioral health outcomes.
Primary Responsibilities:
- Triage individuals to determine what program(s) are
- Complete Program Specific Assessments, which includes administering of screenings, evaluations, comprehensive clinical assessments, and various psychological tests to help support assessment and diagnosis of clients
- Provide treatment, rehabilitation, and follow-up of individuals through individual, couples, family, and/or group psychotherapy services (note: telehealth sessions may be required)
- Complete all documentation within required timeframes, which includes intake paperwork, assessments, progress notes, treatment plans, quarterly reviews, transition of services, etc. Other documentation may be required
- Develop comprehensive treatment plan that is person-centered and focuses on presenting needs while containing frequency, duration, and measurability
- Monitor, revise, and update (with progress, lack of progress, and continuing needs) treatment plans each quarter
- Provide follow-up with "no shows" and ensure individuals make every effort to attend therapy sessions
- Administer suicide screening protocol as needed, referring when necessary; and participate as a member of the agency in the event of any other kind of emergency
- Provide consultation to and coordinate the care of the individual with outside entities (including, but not limited to, primary care physicians, psychiatrists, other behavioral health agencies, social services, ). Identify, refer, and advocate for clients needing specialty behavioral health services, and other services as needed. Participate in multidisciplinary treatment planning and case reviews
- Participate in yearly agency trainings and recertification
- Completion of all CEU requirements and submission of renewal of license as required by the appropriate licensing board. For Residents in Counseling or Supervisees in Social work – actively engage in and follow all guidelines as outlined by the respective boards to earn licensure within expected timeframe.
- Attend staff meetings, team meetings, clinical meetings, supervision, training, and other meetings as required
- Complete other duties as assigned
Knowledge, Skills and Abilities:
- Ability to read and implement VADBHDS/DMAS regulatory standards
- Strong clinical background in evidence-based practices
- Strong psychosocial assessment and treatment planning skills
- Ability to build and sustain relations in the community
- Knowledge of community mental health and public health in order to provide advocacy and enhance an individual’s treatment and access to community resources
- Communicate effectively in order to facilitate treatment and relay information to others as part of an interdisciplinary team
- Ability to assist with training of other staff on program procedures, workflow,
- Ability to communicate in large public settings
- Minimal supervision regarding use of time and able to prioritize work assignments
- Ability to communicate effectively with professionals verbally and in writing with clients/families
- Ability to make sound decisions in emergency situations
- Working knowledge and or experience with Crisis Stabilization services
