Primary Responsibilities:
Qualified Mental Health Professionals are required to assist in the development and coordination of Individual Service Plans (ISPs) and a variety of interventions. The activities associated with this primary responsibility by the Qualified Mental Health Professional (QMHP) are:
- Assist in the initial development and ongoing revision of the Individual Service Plan
- Monitor implementation of Individual Service Plan
- Fulfill case management functions and provide linkage to resources
- Submit accompanying documentation based on required timeframes
- Maintain continuous contact with the identified individual and other key stakeholders of the individual (with signed authorization to release information) in planning and service delivery
- Complete other duties as assigned
Qualifications:
- Bachelor’s degree or higher degree in the Human Service field
- Valid registration with the Virginia Board of Counseling as a Qualified Mental Health Professional-Adult (QMHP-A), Qualified Mental Health Professional-Trainee (QMHP-T), or QPP (must have certificate)
- Must successfully complete a criminal background and central registry check
- Valid Driver’s License
- Current auto insurance
- Current TB screening
- Current CPR and First Aid certification
