Job Summary:
The Substance Use Case Manager will assist individuals and their family members in accessing needed medical, psychiatric, psychological, social, educational, vocational, recovery, and other supports essential to meeting the individual's basic needs. Substance Use Case Management services are to be person centered, individualized, and culturally and linguistically appropriate to meet the individual's and family member's needs.
Primary Responsibilities:
- Assess needs and plan services, to include developing a substance use case management individual service plan (ISP)
- Enhance community integration opportunities and link individuals to community supports
- Assist the individual directly to locate, develop, or obtain needed services, resources, appropriate public benefits and transportation as needed
- Assure the coordination of services and service planning within a provider agency, with other providers, and with other human service agencies and systems, such as local health and social services departments
- Monitor service delivery through contacts with individuals receiving services, service providers, and site and home visits to assess the quality of care
- Advocate for individuals in response to their changing needs, based on changes in the ISP.
- Act as the primary point of contact for individuals and the Interdisciplinary Care Team (ICT), lead ICT process and meetings, facilitate communications among relevant parties regarding individuals’ attendance, drug test results, etc.
- Assess for appropriate changes or additions to services, facilitates referrals for the members, and ensures the ISP is updated as necessary
- Attend and participate in required meetings
- Complete all required/accompanying documentation
- Maintains confidentiality and security of Protected Health Information (PHI)
- Complete additional duties as assigned
Knowledge, Skills, and Abilities:
- Knowledge of substance use disorders, co-occuring disorders, and ASAM
- Basic understanding of psychotropic medications effects and potential side effects.
- Knowledge of different types of functional assessments and their use in services planning
- Knowledge of the signs, symptoms, and effects of substance use disorders, mental illnesses and program philosophy for service provision
- Knowledge of goal development and case management practices
- Knowledge of state and community resources available to individuals
- Knowledge of human rights and confidentiality laws
- Knowledge, skills, and abilities necessary for service planning, care coordination, and documentation
- Ability to work independently as well as function as a team member, maintaining cooperative, effective work relationships
- Ability and skills to establish and maintain ongoing supportive relationships
- Skills necessary to problem solve, observe, record, report, and follow directions
- Skills to manage time appropriately and communicate effectively orally and in writing
Experience and Professional Qualifications:
- Bachelor’s degree in psychology, social work, vocational rehabilitation, counseling, or other VA DMAS approved Human Services degree, or a certified psychiatric rehabilitation practitioner in VA required or Master’s degree in psychology, social work, or counseling from an accredited college or university, preferred.
- At least one year of experience within the past seven years providing substance use or mental health services preferred.
Training:
- Staff must be currently certified in: CPR, First Aid, and verification of Tuberculosis screening.
Background Investigation:
- Staff must successfully complete a criminal background records check conducted by the DBHDS criminal background investigation unit.
- Staff must also have a central registry check completed and must not have any history of positive findings.
