What You’ll Do

As a Care Coordinator, you’ll help patients navigate their healthcare journey, collaborating with a multidisciplinary team to ensure seamless care delivery. Your day-to-day responsibilities will include:

• Patient Navigation: Schedule appointments, complete medication requests, and manage referrals to specialists.
• Care Coordination: Work closely with healthcare providers and community resources to ensure patients receive comprehensive, integrated care.
• Accurate Recordkeeping: Update and maintain precise patient demographic information in the EHR system.
• Triage Support: Direct patients to the most appropriate care based on urgency and established protocols.
• Customer Service Solutions: Address patient concerns and develop effective solutions with the guidance of your team.
• Continuous Improvement: Participate in quality improvement activities to enhance patient outcomes and care delivery.
• Patient Engagement: Send outbound questionnaires via email or text, collaborating with the Care Management team to gather valuable patient insights.

Qualifications

• High school diploma or GED required.
• Experience in patient-centered care coordination, call centers, or a strong desire to help people navigate their healthcare journey.
• Exceptional communication and interpersonal skills, with the ability to build trust and rapport with patients.
• A genuine passion for serving, educating, and advocating for all members of our community, including the most vulnerable.

This is a full-time, on-site position based in the Metro Detroit area. A background check is required for this role.
