Epic Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our care team as a Care Team Leader. In this role, you will oversee the delivery of high-quality rehabilitation and skilled nursing care to residents in a 5-star rated facility.
The successful candidate will possess excellent communication, organizational, and leadership skills. They will be responsible for collaborating with the care team, monitoring patient responses, and leading a team of Certified Nursing Assistants (CNAs) while prioritizing resident needs.

Key Responsibilities:
- Monitor and document vitals, treatments administered, and residents' responses
- Collaborate with the care team to revise/update the Nursing Care Plan or any pertinent findings including lab/test results, incidents, and/or concerns
- Prepare, organize, oversee, monitor, and evaluate delivery of care of resident CNA assignments and activities, considering the category of needs and competency of personnel
Requirements:
- Graduate of an approved school of Nursing
- Currently registered in good standing in New York as an LPN
- Dedicated to providing extraordinary, clinically sound, and compassionate care
- Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health
- Have excellent communication, customer service, and organization skills
