**Job Summary**
We are seeking a dedicated Licensed Practical Nurse to join our team in the Weekend Program at EPIC Rehabilitation and Nursing. As a key member of our care team, you will provide high-quality direct care and support to residents in a skilled nursing facility.
The ideal candidate will possess excellent communication and customer service skills, with a genuine interest in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents. You will work collaboratively with the care team to ensure optimal rehabilitation and health outcomes for our patients.
Responsibilities:
- Administer medications and monitor patient vitals
- Collaborate with the care team to revise/update the Nursing Care Plan or any pertinent findings
- Monitor and document vitals, treatments administered, and residents' responses
Requirements:
- Graduate of an approved school of Nursing
- Licensed and currently registered in good standing in New York as an LPN
- Dedicated to providing extraordinary, clinically sound, and compassionate care
