Key Responsibilities
As a Travel - Physical Therapist - Home Health: You will provide patient care in accordance with Physical Therapy Standards of practice, working with adult and geriatric patients with various orthopedic, neurological, and functional mobility diagnoses.
Requirements:- WI license
- 1 year experience
- Home Health experience
- Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy
- Master's Degree in Physical Therapy required
- AHA BLS
- Float assignment possible
- Must obtain NPI number if candidate does not have it already
- Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
- Documents to meet regulatory and reimbursement requirements
- Meets productivity expectations of an average of 4.5 visits per day
- Scrub Colors: any or Business casual
- Must live outside 50 miles
