Requirements
- VT License
- 3 years of prior community hospice experience
- Symptom assessment and management with high level of critical thinking skills
- AHA BLS
- Hospice/End of Life-Complex symptom management and education
- Comfort initiating and participating in goals of care discussions and planning with patients, family, and team
- Medication management and administration (IV, Subcutaneous, oral, sublingual, rectal, and other routes
- Covid and Flu declinations accepted
