Job Summary
Through Tailored Care Management, Behavioral Health Tailored Plan beneficiaries will have a designated care manager supported by a multidisciplinary care team to provide whole-person care management that addresses all their needs, spanning physical health, behavioral health, pharmacy, long-term services and supports (LTSS), and unmet health related resource needs. The Care Manager Extender will assist in meeting the needs of the member, create additional workforce capacity, and allow care managers and supervisors to focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs.
Responsibilities
- Share personal story to assist/support the member.
- Performing general outreach, engagement, and follow-up with members.
- Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation).
- Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing.
- Sharing information with the care manager and other members of the care team on the member’s circumstances.
- Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.
- Participating in case conferences.
- Support the care manager in assessing and addressing unmet health-related resource needs.
- Regular communication and coordinate efforts with Care Manager and Care Manager Supervisor to decrease duplicative and inappropriate outreach to assigned members.
- Other duties as assigned.
Qualifications
- Is at least 18 years of age.
- Has a high school diploma or equivalent.
- Is trained in Tailored Care Management (as described later in this document).
- Is supervised by a TCM Care Manager and meets one of the below requirements: 1. Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist 2. Has one year of paid experience performing the types of functions described in the “Extender Functions” section below.
Individuals who will be able to meet these qualifications, including, but not limited to the following:
- Certified Peer Support Specialists; or
- Community health workers (CHW), defined as individuals who have completed the NC
- Community Health Worker Standardized Core Competency Training (NC CHW SCCT); or
- Individuals who served as Community Navigators prior to the implementation of Tailored Plans
Knowledge, Skills, and Abilities:
- Person-Centered Thinking/planning
- Knowledge of using assessments to develop plans of care.
- Knowledge of LOC processes
- Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans
- Familiarity with or intent to learn the use of Motivational Interviewing techniques.
- Strong interpersonal and written/verbal communication skills.
- Conflict management and resolution skills
- Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)
- High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance.
- Ability to make prompt, independent decisions based upon relevant facts.
- Good organizational skills to prioritize duties and work with minimal levels of onsite supervision to consistently meet deadlines.
