Pay: $31.20 - $36.78 per hour
Job Summary
The Community Outreach Associate performs a variety of supportive services with the focus on facilitating effective communication and sharing of information between departmental staff and external customers and other departments. Responds to referrals around services for patients in the TOC scope of services. Conducts follow-up phone calls within 24-48 hours post discharge. Ensures patient's care is coordinated and meets post discharge needs. Makes Home/Community visits to patients as needed. Triages calls to appropriate team members. Assists patients with completing applications for entitlement benefits.

Requirements
- Bachelor's Degree in Social Work or Health related field required
- Bilingual English/Spanish required
- Experience working in a health care office or setting
- Knowledge of health insurance information and health terminology
Salary : $31 - $37
