Minimum Qualifications:
- Bachelor’s Degree in social work or other related field preferred. A minimum of 4 years’ experience in a directly related field will be considered in lieu of a degree.
- Prior experience working with the senior or aging population recommended.
- Moves equipment and supplies weighing up to 25 lbs on a regular basis and up to 50 lbs on an occasional basis. May push residents in wheelchairs as needed.
- Able to read, write and communicate effectively in the English language.
- Able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met.
- Able and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, residents, family members, volunteers, government agencies/personnel, and the general public in a fast-paced work environment.
- Have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to work with residents based on whatever maturity level in which they are currently functioning.
As a Service Coordinator you can expect to:
- Provides general service management, which includes intake, education (services available and application procedures) and referral to service providers in the general community. Ensures all residents requiring assistance are referred to the appropriate agencies that provide the needed services and helps them obtain benefits for which they are eligible.
- Serves as a liaison with residents, family members, social agencies, and other ERS team members as appropriate, concerning resident needs and/or concerns; networks with community providers and seeks out new services available to the residents.
- Is responsible for maintaining adequate case files on all residents requiring social support services. Completes and submits reports or other supportive documentation as requested or required.
- Assesses residents’ needs for services and assists residents/families to resolve social and environmental problems including but not limited to: financial needs, including providing information on Medicare/Medicaid, problems related to clothing and possessions, transportation needs, relationship difficulties. Provides information and refers to internal or community resources as needed. These social services may include Meals on Wheels, health aides, homemakers, counseling, preventative health screening, and other needed services.
- Responsible for providing each resident with an assessment shortly after his or her time of move-in and at least annually thereafter.
- Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate.
- Assists the residents in building informal support networks with other residents, family and friends. Encourages residents to volunteer and participate in activities.
- Conducts research on a regular basis. Analyzes the types, frequency and other characteristics of services that residents use, need and want. Reports this information to the appropriate management team members as requested.
- Arranges and coordinates for various speakers and service providers to come to the community on a regular basis to promote health, wellness, and benefits as appropriate as well as other issues of interest and/or need to the residents. Assesses residents’ participation in, and satisfaction with programs, interest in new programs and barriers to greater participation.
- Assists in reviewing complaints and grievances made by the resident/family and makes a written/oral report to the Director of Resident and Health Services.
- Completes and submits incident reports within the required timeframe.
- Tracks and reports various metrics and information (e.g., hospitalizations and others) as requested by the Director of Resident and Health Services.
- Maintains a resource directory. This directory may include a listing of State and/or local service providers that residents can contact for assistance.
- Keeps abreast of current federal, state, and local regulations, and makes recommendations on changes in policies and procedures to the Community Manager.
