Your Key Responsibilities:
- Coordinate and implement individualized care plans, revising as necessary.
- Educate patients and their families on disease processes, self-care.
- Initiate preventive and rehabilitative nursing procedures.
- Document clinical notes and patient progress to meet established goals.
- Collaborate with the interdisciplinary team to prioritize and deliver care effectively.
- Supervise LPNs and Home Health aides, ensuring quality standards are met.
- Provide on-call coverage as part of a rotation schedule.
What We’re Looking For:
- Education & Licensing: Graduate of an accredited nursing program with a current RN license in OK.
- Experience: 1-2 years of recent nursing experience (home health experience preferred).
- Skills: Strong communication, critical thinking, and interpersonal abilities.
- Certifications: Current CPR/First Aid certification from the American Heart Association.
- Physical Ability: Capable of occasionally exerting 50-100 pounds of force.
- Transportation: Valid driver’s license, reliable vehicle, and proof of insurance.
