In this vital role, you’ll be the bridge that connects patients to high-quality home health and hospice care, ensuring a smooth transition during an important chapter of their lives. 💼 Schedule : Monday - Friday, 8 AM to 5 PM 💰 Starting Pay : $30-$35 / hour (DOE) What You’ll Do As the LPN Intake Coordinator , you’ll play a critical role in ensuring patients receive the care they deserve. Your responsibilities include : Reviewing and managing incoming referrals to ensure compliance with home health / hospice requirements. Coordinating schedules to deliver timely, high-quality care. Communicating with physicians, referral sources, and care teams to address patient needs. Ensuring accurate documentation, billing coordination, and compliance with regulatory standards. Assisting with direct patient care as needed and participating in on-call rotation. Who You Are We’re looking for someone who is : Qualified : Licensed as an LPN with a nursing degree from an accredited school. Experienced : 2 years in home health or hospice preferred. Customer-Focused : Skilled in patient relations and compassionate care. Organized & Detail-Oriented : Able to handle referrals, scheduling, and compliance with ease. Prepared : Holds a valid driver’s license, reliable vehicle, and insurance to meet job requirements.
