Job Summary and Responsibilities


Acentra Health is looking for a Clinical Assessor to join our growing team in New Hampshire.


Job Summary:

As a Clinical Assessor at Acentra Health, you will play a vital role in supporting your community by conducting comprehensive, needs-based eligibility assessments for patients applying for Medicaid-funded personal care services. These evaluations, completed in settings such as nursing homes, hospitals, and community-based locations, are essential in determining the appropriate level of care and ensuring patients receive the support they need to enhance their well-being and quality of life.


In this role, you will provide critical preadmission screening and resident review (PASRR) Level II evaluations for individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC). Your work will directly influence care planning, placement decisions, and the facilitation of next-level care for those in need. Responsibilities:
  • Conduct thorough clinical assessments of patients, reviewing relevant records and consulting collateral sources to ensure accurate evaluations and well-informed recommendations.
  • Recommend appropriate levels of care and service needs based on a comprehensive evaluation of the patient’s physical, behavioral, and psychological functioning.
  • Complete assessments promptly and within the scope of practice, meeting all contractual and regulatory requirements.
  • Demonstrate strong typing and computer skills, with proficiency in Microsoft Word, Outlook, and other relevant software.
  • Adhere to program workflows and ensure compliance with established processes and standards.
  • Perform person-centered assessments, providing meaningful and well-considered recommendations for levels of care and support services.
  • Leverage clinical expertise and knowledge of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM) applicable in New Hampshire.
  • Apply knowledge of mental health standards of practice, as well as intellectual and developmental disabilities, geriatrics, and conditions commonly requiring long-term care services and supports.
  • Collaborate effectively with healthcare professionals, patients, families, and community support systems to ensure optimal care and outcomes.
  • Communicate clearly and effectively with individuals and groups using verbal, written, and electronic methods.
  • Maintain a high level of attention to detail, prioritize tasks effectively, and execute responsibilities in a timely and efficient manner.

Qualifications

Required Qualifications, Experience, And Knowledge

  • Active, unrestricted RN license with eligibility to practice in New Hampshire.
  • Graduation from an accredited college or university with a degree in a relevant field, as required for this position.
  • Proficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processes.
  • Demonstrated expertise in clinical assessment and critical thinking, ensuring accurate evaluations and recommendations.
  • Exceptional verbal and written communication skills, with the ability to convey complex information effectively.
  • Proven ability to work collaboratively within a team environment, fostering cooperation and shared goals.
  • Flexibility to adapt to changing priorities and strong organizational skills to manage multiple tasks effectively.
  • Experience with public mental health and medical programs, as well as public sector services and supports.
  • Proficient with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, Outlook, and other web-based software.
  • Keen attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely manner.

Compensation

The compensation for this role is $31.00 to $34.00 per hour

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
