Acentra seeks a Clinical Evaluator to join our growing team in Sacramento, California.


Job Summary:

As a Clinical Evaluator PASRR for Preadmission Screening and Resident Review, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in Sacramento, California. Your role involves providing essential evaluations to enhance patient well-being and improve their quality of life. These assessments, primarily conducted in nursing homes, hospitals, or community-based settings, play a crucial role in determining the level and scope of care required for each patient.


As a trusted expert, you will evaluate, determine, and recommend the next level of care to improve overall quality of life. This is a (Full-Time) Hybrid role, combining work from your home office with travel to evaluate patients in your local area, primarily within Sacramento area. Travel locally to perform essential evaluations, with all travel expenses, including mileage and parking, fully reimbursed. These Preadmission Screening and evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.


Job Responsibilities:
  • Conduct PASRR screenings, evaluations, and determinations through comprehensive clinical assessments of patients, including review of relevant records and input from collateral sources.

  • Complete precise PASRR reviews by assessing and determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations Assess and analyze psychological, emotional, behavioral and physical functioning.
  • Ensure full compliance with established workflows, including re-evaluations of records based on psychiatric input.

  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Provide well-informed level of care recommendations based on clinical evaluations.
  • Collaborate effectively with healthcare professionals, patients, families, and internal stakeholders as needed.
  • Efficiently utilize computer equipment and web-based software applications to perform job duties
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities. .
  • Efficiently prioritize and execute tasks with attention to detail.

Qualifications


Required Qualifications, Knowledge, and Experience

  • Must possess an active, unrestricted state licensure in a healthcare or related field, such as Licensed Psychologist, Licensed Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker, Social Worker Independent Clinical License, Nurse Practitioner, or Psychologist.

  • Graduation from an accredited program with a Diploma, Associate’s Degree, or Bachelor’s Degree in a relevant field.

  • Minimum of 2-3 years of experience in the assessment and diagnosis of individuals with mental/psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.

  • Minimum of 1 year of experience working with individuals with intellectual disabilities or related conditions.
  • Familiarity with standards of practice in mental health, intellectual and developmental disabilities, geriatrics, and other disorders requiring long-term care services.
  • Excellent verbal and written communication skills.

Preferred Qualifications

  • Knowledge of Federal Regulation 42 CFR, Chapter 483.

  • Proficiency in diagnosing psychiatric disorders as outlined in the most current Diagnostic and Statistical Manual (DSM) applicable in the state.

  • Understanding of standards of practice related to Level of Care determinations.

  • Proficiency in medical record abstracting, including knowledge of medical record organization, medical terminology, and disease processes.

  • Experience working with the geriatric population is preferred.
  • Ability to communicate clearly and effectively with individuals and groups through verbal, written, and electronic media.

  • Strong clinical assessment and critical thinking skills.

  • Strong attention to detail with the ability to prioritize tasks and execute them efficiently within deadlines.

Compensation

The compensation for this role is $85,280.00 to $87,360 base salary

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
