About the Role
- This 13-week contract position offers 34 hours per week, with a shift of 8 hours, days.
- The successful candidate will be responsible for completing medical record documentation, maintaining medical records, and participating in assigned meetings.
Social Worker Responsibilities:
- Securing collateral information from various sources.
- Maintaining ongoing contact with patient's family and significant others.
- Assisting families in finding support services and engaging families in the treatment process.
Requirements:
- Candidate must be a licensed as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- Able to work independently and collaboratively as part of an interdisciplinary team.
