What are the responsibilities and job description for the Case Management Nurse position at Equity Staffing Group?
We are looking for an Acute Transitional Care Nurse (RN) for a 6 month assignment. Our client, serving over 85 million people, is known for bold ideas and a collaborative environment. If you're passionate about working with energetic people, we want to hear from you!
This is a telecommute role so must leave in one of the following areas Tampa or Orlando area.
Essential Functions:
- Prioritize patient care needs and address emerging issues
- Meet with patients, families, and caregivers to discuss care and treatment plans
- Identify and assist with patient follow-up in various care settings
- Optimize clinical outcomes using established protocols and resources
- Oversee discharge provisions, including follow-up appointments and home health services
- Maintain effective communication with healthcare teams, patients, and families
- Demonstrate understanding of cost consequences and utilization of reports and systems
- Consult with physicians and team members to implement care plans
- Monitor and coach patients using motivational interviewing and behavioral change techniques
Requirements:
- Bachelor's degree in nursing (or Associate's degree with 4 years of experience)
- Active and unrestricted RN license in Florida
- Minimum 2 years of applicable clinical experience in a physician's office, clinical, or hospital setting
- Valid Florida driver's license and proof of insurance
Preferred Qualifications:
- Comprehensive care continuum management, utilization review, or discharge planning experience
- HMO experience
