Position: Inmate Health Services Navigator
Location: 134 W. Eagle, Buffalo, NY 14202
Shift: Monday - Friday/8a-4pm
Minimum Qualifications: Graduation from a regionally accredited or New York State registered four (4) year college or university with a Bachelor's degree and one (1) year of office clerical experience;
Obtain the New York State Department of Health IPA/Navigator certification within one (1) month of employment.
Must be able to pass background investigation.
Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements.

It is your responsibility to monitor civil service exam announcements and sign-up for the exam. Willful failure to compete will result in termination of provisional appointment. Service under provisional appointment does not confer or establish any prior right toward any permanent appointment. To receive a permanent appointment, you must be reachable on the eligible list when it is established. Also, as a provisional employee, you will begin to accrue paid sick leave after six months of continuous service. **
OVERVIEW
The work involves performing a variety of clerical duties, interacts and performs interviews with identified inmates within the confines of the Jail Management Division (JMD). The IHSN collaborates with the Erie County Correctional Health Division (CHD) to identify inmates that are at high risk for required medical care. The IHSN will review the current inmate status and meet with identified inmates to facilitate enrollment with Medicaid and identify inmates that require Medicare. The IHSN collaborates with the Erie County Department of Social Services to facilitate necessary enrollment. Work is performed under the direct supervision of a higher-ranking employee designated by the Erie County Sheriff’s Office. Supervision is not a function of this position. Does related work as required
TYPICAL WORK ACTIVITIES
- Responsible for the collection of inmate information, documentation of inmate’s status within the electronic health records system, review of budget items related to inmate care as established through the navigation process;
- Works directly with inmate population to provides a comprehensive knowledge base of the NYS health insurance program for Medicaid;
- Assists the inmates by completing their enrollment application in order to receive health care coverage;
- Receives inmate medical information, documents regarding inmate’s medical chart and insurance required along with the State approval or Denial of such health insurance electronic medical records (EMR) system;
- Provides culturally competent and accessible inmate assistance;
- Maintains and monitors the Bravo Reception Report for all new inmates booked into the Erie County Holding Center, collaborates with designee from CHD to identify inmates by risk factor and reviews for necessary healthcare coverage;
- May develop a policies and procedures/standards pertaining to healthcare requirements and training within the Erie County Holding Center and the Erie County Correctional Facility;
- Collaborates with security to schedule face to face inmate meetings;
- If inmate is identified as sent to hospital for emergency services the IHSN will collaborate with hospital identified to facilitate enrollment;
- Develops and reviews statistics and inmate data for inclusion in records, reports and other documentation utilizing EMR databases;
- Types forms, records and other documents relating to inmate enrollment; Attends related training sessions and seminars;
- Collaborates with the CHD for inmate internal audits/reports to establish appropriate invoices verse services rendered
KNOWLEDGE, SKILLS, and ABILITIES
Thorough knowledge of New York State Medicare and Medicaid eligibility, enrollment, and program specifications; good knowledge of HIPAA guidelines and security standards; good knowledge of guidelines, laws and regulations related to patient privacy and protected health information; good knowledge of clerical procedures; working knowledge of security requirements and organizational structure of a correctional facility; ability to compile and maintain information and records; ability to prepare reports; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications including but not limited to electronic health records and Microsoft Word and Excel; ability to work independently and make decisions in compliance with organizational standards; clerical aptitude; sound professional judgment; neatness; tact; confidentiality; courtesy; dependability; patience; capable of performing the essential functions of the position with or without reasonable accommodation.
