Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Care Management Team ! The Licensed Care Manager ensures the delivery of comprehensive, high-quality, patient-centered care management services for Erie Family Health Centers (Erie's) high risk patients. Key responsibilities include conducting face-to-face and telephonic patient interactions, completing risk assessments, developing care plans to directly impact patient outcomes, helping patients learn to manage their conditions and providing needed psychosocial resources for a panel of 75 patients.
