Description
Erie Family Health Centers is looking for a compassionate individual who is interested in community outreach and health education like you to join our Community Health Team! The Health Educator works independently and as part of a team to support internal and external community health programs.
Responsibilities
- Conduct educational appointments with patients, preparing for and facilitating group sessions, providing patient follow-up and presenting educational workshops that focus on nutrition, diabetes, disease prevention and physical activity
- Provides training and assistance for staff regarding health education issues
- Acts as member of patient care team with clinical staff and other health care providers
- Strong oral communication skills and can communicate effectively with patients, health care providers and community members
Qualifications
Requirements
- Associate degree in Community Health or Health Education (or equivalent of education and directly related experience)
- One or more years working in a community health setting preferred
- Experience conducting individual or group education presentations
- Strong Interpersonal and oral communication skills
- Ability to communicate effectively in both English and Spanish required (May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests)
- Demonstrated cultural competency to work in a low income, minority environment preferred
