Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Lending Hands for Life, Patient Programs and Support Services! The LHL Coordinator is responsible for leading patient supportive services for Prep and Hepatitis C care at Erie Family Health Centers. The LHL Coordinator provides supervision and leadership to a team of Community Health Workers and a service year member, Primary duties include being the point person for new patients to LHL programs, doing quality assurance and reporting for the programs, and ensuring an effective and productive team delivering quality care.

*Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Main Duties & Responsibilities
Supervision Responsibilities include but not limited to:
- Hires, onboards, and trains employees.
- Manages Team schedules and time off requests.
- Supports creation of onsite and case conference schedules, ensuring coverage.
- Completes scheduled performance evaluations and 1:1 meetings with team.
- Facilitates monthly meetings and periodic trainings for team.
- Communicates all new initiatives to team members and sites to clarify questions and ensure smooth rollout.
- Ensure implementation of agency policies and procedures.
- Support service year member with LHL Special Projects including: World AIDS Day, Cooking Class, Summer Social, etc. Be lead on AIDS Run/Walk fundraiser.
PATIENT CARE, QUALITY ASSURANCE, and REPORTING OUTCOMES
- Completes new patient intake and program orientation for new clients and links with appropriate resources.
- Create protocols for programs. Train team on protocols.
- Maintains active patient registries of all patients in program(s). Involved in maintaining patient retention in the program.
- Ensures PrEP injectable registry is maintained, and patients are up to date on needed care.
- Monitors and responds to messages in PrEP/HCV pool.
- Responsible for quality assurance. Does periodic chart review and audits of team members’ cases.
- Creates monthly outreach and eligibility lists for team members.
- Focuses on quality and outcomes of programs. Works with team and providers to identify and implement process improvements.
- Responds as needed to funder organizations’ requests and reporting requirements, including HIV reports.
- Ensure compliance with funder requirements.
OTHER DUTIES
- Create presentations, summaries and other communication materials to ensure better communication and agency-wide understanding of the program.
- Identifies outreach opportunities to expand programs and reach new patients.
- Engages with community partners on collaborations or referral resources.
- Attends funder and partner meetings, trainings, conferences as directed.
- Attends all required internal meetings including case conferences, site-based meetings, team meetings, etc.
- Participates in internal and external committees, as directed.
- Monitor program budget and track expenditures.
- Cross-train with other LHL Coordinator and be familiar with other content area(s) of LHL Department.
- Provide coverage during staffing gaps/transitions.
- Promotes a positive image of Erie Family Health Center with community partners.
- Performs other duties as assigned.
Qualifications
Education
- High school diploma; with 4 years of progressively responsible leadership experience
- Associate degree or two years of college; with 3 years of progressively responsible leadership experience
- Bachelor’s degree from a 4-year accredited University; with 2 years of progressively responsible leadership experience
- Master’s degree; with 1 year of progressive leadership experience
Skills and Knowledge
- Bilingual fluency (English/Spanish) strongly preferred
- Computer and data entry skills
- Ability to establish and maintain effective working relationships with co-workers, clients and community partners
- Ability to plan, organize, implement, evaluate, and modify program needs
- Ability to independently program solve and address issue as they arise
- Ability to work with staff members across all levels of the agency
