Erie Family Health Centers, a top workplace in Chicago and the suburbs, is looking for a valuable addition to our Patient Program and Support Services! The Population Heath Outreach Program supports patients by phone and works mainly remotely with clinical care team members across all sites. As part of the role, the Population Health Coordinator helps improve the agency’s quality of health care through targeted, date driven outreach. Outreach is based on agency priorities, including chronic disease and preventive health needs and specialty care referral navigation, and includes a variety of methods such as campaign texting, 1on1 patient phone calls, bulk referral ordering, MyChart, as well as others. The team also assists patients with any health-related social needs that presents as barriers to care.

 

 

The Population Health Coordinator role provides supervision, training, and helps monitor productivity and performance of team members. They are responsible for using HER, MCO portals, and other data sources for driving agency priorities and team work to help achieve program and agency goals.

*Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

 

 

Main Duties & Responsibilities

  • Hires, onboard, and trains new employees, volunteers, service members and interns. Manages team, schedules, and time off requests.
  • Completes scheduled performance evaluations and 1:1 meetings with team members.
  • Assists Senior Manager in facilitating monthly meetings and regular training for full team. Tracks productivity and performance of team members and ensures that individual team members meet performance metrics.
  • Develops and refines workflows, protocols, reports, and tools to ensure continuous quality improvement of outreach to address care gaps and health-related social needs.
  • Builds effective relationships with local social service agencies and advocates for clients in the process. Promotes a positive image of Erie Family Health Centers with community partners.
  • Attend funder and partner meetings, trainings, conferences as directed.
  • Collaborates with healthcare providers (such as primary care providers, behavioral health team, nurses, and other Patient Programs staff) to ensure high quality care for Erie patients.

Qualifications


Education

  • High School diploma or equivalent required
  • Associate college degree or 4- year college degree preferred.

 

 

Skills and Knowledge

  • A minimum of four years of work experience in health, social or community services or comparable experience required.
  • Experience in a supervisory role required.
  • Bilingual in fluency (English/Spanish) a plus
  • Strong computer skills required, expert in Microsoft Office. Experience with Epic electronic health record system strongly preferred.
  • Local travel may be required for the position.
