The Associate Director of Patient Programs – PCMH and Quality Analytics position is responsible for ensuring data-driven population-level quality improvement programs are designed and implemented in a manner that aligns with providing effective, high-quality, and patient-centered care.

- Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
- Project manage PCMH accreditation process and ongoing maintenance.
- Be the agency lead on NCQA content, including EHR-related content, reporting, and standards.
- Develop and maintain strong collaborative relationships with agency leadership, care teams, and all other stakeholders to ensure open communication, the sharing of information, the examination of current clinical and operational workflows, and implementation of PCMH-focused changes and improvements.
- Measure and evaluate data to determine the ultimate impact of the programs on patient health outcomes and revenue implications, ensuring adequate organizational workflows are in place to support outcomes.
- Through reporting and chart review, identify process gaps, trends, and disparities related to UDS and HEDIS performance metrics, as well as misalignment of payor and agency data.
- Perform other duties as assigned.
Education
Required:
- A bachelor’s degree in public health, business, or health administration is required.
- A master's degree in health care, social work, public health, or business administration is preferred.
Required:
- Having successfully gone through the PCMH recognition process.
- Highly proficient with Microsoft Office, including Microsoft Excel.
- NCQA PCMH Content Expert Certification (CEC).
- Project management experience.
- Experience using Epic Software a plus.
- Quality Improvement or Process Improvement trainings or certification (ex. Lean Six Sigma, Institute for Healthcare Improvement, etc.) a plus.
- Implement change within Erie Family Health Centers.
- Objectively evaluate and document program effectiveness.
- Integrate Programs with other services provided through the Health Centers.
- Ability to develop and track budgets.
- Ability to plan, organize, implement, evaluate, and modify program needs.
