The Licensed Care Manager ensures the delivery of comprehensive, high-quality, patient-centered care management services for Erie Family Health Centers (Erie’s) high risk patients. Key responsibilities include conducting face-to-face and telephonic patient interactions, completing risk assessments, developing care plans to directly impact patient outcomes, helping patients learn to manage their conditions and providing needed psychosocial resources for a panel of 75 patients.

- Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
- Provides care management services and monthly follow-ups for a panel of 75 patients consistent with contractual and Erie requirements for quality and service excellence.
- Collaborates with patient programs, supervisor, providers, and care teams as well as uses Epic reports to identify appropriate patients for care management and identify internal support programs and services.
- Works with patient and patient’s care team to develop, document and monitor each patients’ individualized treatment care plan.
- Conducts chart reviews, utilizes electronic medical record, conducts brief and more comprehensive health risk assessments, motivational interviewing and medication reconciliation to identify patients’ clinical concerns and acuity, risk factors, and barriers to achieving plan goals.
Education
- Minimum Associate’s degree with 3 years nursing experience required
- Current Illinois RN or LPN licensure required
- Bilingual fluency (English/Spanish) preferred. (May be required to assist or work with non-English/Spanish only speaking patients and/or guests.)
- Strong computer skills: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).
- Active CPR card required
- 1 year experience as a licensed professional preferred
- Experience working in community health or safety net settings preferred
- Medical or mental health case management experience preferred
