Erie Family Health Centers is seeking a part time Community Health RN Float to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills.

Main Duties & Responsibilities
- Collaborates with providers in the delivery of patient care.
- Performs triage assessment process of walk-in patients and telephone calls.
- Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
- Administers immunizations, applicable contraception refills, and medications as ordered by provider.
- Performs other duties as assigned.
Education
- Current Illinois RN licensure required.
- Current BLS for Healthcare Provider CPR certification required.
- One to two years’ experience in community health nursing or ambulatory care preferred.
- Bilingual is a plus.
