SUMMARY
Non Medical Transportation (NMT) Driver is responsible for providing transportation to individuals with developmental disabilities to and from day programming and/or employment throughout the week. May also assist with outings in the community throughout the week and occasional weekends, transportation to doctor’s appointments throughout the week, and for shopping needs for the individual within our group homes. As a Direct Support Professional (DSP), will assist individuals referred from any of ERLI’s referral sources to meet the basic goals assigned in their plans both in the community and/or group home, as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Non Medical Transportation Duties
Driver safely provides transportation for individuals with developmental disabilities to and from day services and community sites while driving a non-CDL rated vehicle.
Complete daily trip check prior to each route. Note all service needs to Program Supervisor.
Assists passengers on and off the vehicle (as needed) and work with residential staff or ERLI staff who may be assisting to load and unload passengers
For wheelchair bound passengers, driver will secure the wheelchair to restraining devices to stabilize wheelchair during trip according to standards outlined in trainings.
Must know advance trip destination by routing / charting maps. Must follow designated route unless change is reported to Transportation Manager.
Must report to Program Supervisor via provided cellular telephone any emergencies, behavioral concerns or disruptions of service.
Must maintain the highest level of customer service with the participants at all times.
Must complete a detailed cleaning of vehicle bi-weekly and manage the servicing of the vehicle with fuel, lubricants, and accessories.
Must remove all trash and sweep (as needed) vehicle at the end of each day
Maximize gasoline usage by following fixed routes.
Responsible for parking & securing vehicle at the end of shift.
Must record mileage and destination information in the Drivers Trip log, daily. Additionally, all insurance, safety and vehicle maintenance information will be kept in an organized and accessible manner on the vehicle.
Direct Support Professional Duties
Develop and implement programming designed to increase daily living skills, personal and social behaviors.
Assist in ensuring that individuals develop and understand important independent living skills such as attendance, punctuality, planning, grooming and hygiene and activities of work and daily living.
Assist individual in engaging in programming designed to increase daily living skills and personal and social adjustment.
Develop a schedule of activities that meets the individuals’ program of service, areas of interest, Individual Service Plan (ISP) and person-centered plan.
Develop, implement, and document community engagement and vocational activities (i.e. outings in the community) weekly and/or bi-weekly dependent on the ISP and person-centered plan for individuals supervised.
Using electronic record keeping system to write and develop detailed, accurate, and SMART goals and case notes on activities for each individual assigned to your supervision. Electronic records must follow Medicaid and CARF requirements.
Maintain caseload documentation of case notes, ISP revision dates, documentation, recommendations for transition to next opportunities etc. and keep supervision up-to-date regarding individual progress. Caseloads are built based on acuity, agency needs, and individual’s skills and interests.
Provide on-the-job guidance and support to help move the participant toward attainment of targeted behaviors.
Use modeling techniques to instruct individuals and groups in proper work methods and positive work behaviors and how to improve work quantity and quality.
Write monthly reports regarding an individual’s progress.
Participate in annual and/or quarterly ISP meetings for all individuals supervised, as requested by supervisor.
General Duties
Follows all Medicaid rules and CARF standards. Driver is responsible for maintaining all required Medicaid compliances and training.
Ensure that the participant is kept clean and unsoiled, and in the event of an accident, assist the individual in clean-up and changing their clothes if necessary.
Complete all required paperwork for Medicaid billing and submit transportation sheets to Program Supervisor.
Responsible for immediately report any unusual behavior of passengers or with regards to vehicle and follow mandatory reporting guidelines.
Maintain a positive work atmosphere by behaving and communicating in a professional manner that gets along with stakeholders, vendors, individuals, co-workers, and supervisors. This also includes guardians/parents, caregivers, providers and any other agencies you may have contact with.
Ensure all required paperwork, such as monthly reports, Individual plans and documentation, program evaluation forms, detailed and accurate case notes, etc. is completed as required and forwarded to case management staff in a timely manner.
Ensure Unusual and Major Unusual Incidents are documented and reported according to Ohio Revised Code regulations and the health and safety of the participants served is always a priority. Document Unusual Incidents/Major Unusual Incidents (UI/MUI) in case management system. Notification to Supervisor, family, Support Administrator, and others identified required within compliance requirements.
Provide transportation to individuals for appointments, shopping, outings, and so forth in an agency vehicle.
Complete and maintain CPR/First Aid and Delegated Nurse training during entire duration of employment.
Continue personal, professional development by attending and hosting seminars, workshops, and joining professional organizations.
Responsible for following agency safety policies and maintaining a safe work environment.
Maintain regular attendance per Erie Residential Living, Inc. policy.
Maintain a positive work atmosphere by behaving and communicating in a manner that gets along with customers, vendors, individuals, co-workers, and supervisors.
If needed, will provide direct care to individuals, including but not limited to supervising daily activities, ensuring safety and security, and assisting with personal hygiene and self-care tasks. Will maintain a therapeutic environment by utilizing positive behavior management techniques, providing consistent and clear expectations, and promoting a sense of community among individuals.
Other duties as assigned.
