Job Summary:
A registered professional nurse employed by Erlanger Health System is responsible for direct and/or indirect care of the patient from referral to transplantation at Erlanger Health System. Demonstrate leadership, communication skills, flexibility, and creativity while working with transplant patients and their families through all phases of transplantation. The pre transplant coordinator will need knowledge of chronic kidney disease, treatment options, and an understanding of the transplant process. The pre transplant coordinator applies policies and procedures to managing a case load of patients referred for transplant and functions as a resource for patients and staff about the transplantation process. Maintains communication with referring physician's pre and post-transplant. Is an active member of the multidisciplinary Transplant Team. Understanding of UNOS and CMS regulations. Active participate in transplant QAPI. Demonstrate problem-solving abilities as well as the ability to manage and/ or direct various personalities or educational levels in dynamic situations. Demonstrate computer skills and ability to work simultaneously in multiple databases as well as EMR. Exhibit the ability to work effectively and consistently in a group as part of a high function. Have the ability to work independently as well as a multidisciplinary team member. Demonstrate the ability to handle patient caseloads in an organized manner. Be committed to ongoing personal education in the specialty area. Ability to be cross-trained in transplant related areas.
Education:
Required:
BSN (within three years of hire date)
Preferred:
Certification in specialty preferred
Experience:
Required: At least three to five years of general nursing practice; knowledge of organizational functions, policies, and regulations; knowledge of current trends and new developments, principles and practices of nursing. Knowledge of ESRD and/or transplant patients desired. Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations. Ability to maintain confidentiality with sensitive information or issues in a confidential and respectful manner. Ability to plan, organize, prioritize, multi-task, and meet deadlines. Ability to work effectively with individuals at all levels of the organization. Knowledge of principles and practices of customer service and telephone courtesy. Knowledge of Windows-based office software, computers, and operating system. The ability to defuse stressful situations.
Preferred:
At least one year of experience appropriate to the specialty of transplant. Bilingual skills are a plus.

Required:
Current RN licensure to practice nursing in the State of Tennessee.
BLS completed prior to going to the unit/ department and maintained going forward
The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.

Department Position Summary:
The Pre-Transplant Clinical Coordinator employed by Erlanger Medical Center assures that pre-transplant patients presenting to Erlanger Center receive optimal care, using established renal transplant standards and organizational goals. The functions of the Pre-Transplant Clinical Coordinator are to maintain a high degree of personal and staff effectiveness in the assessment and care of pre transplant patients, to include data management, screening, medication management, discharge planning, clinical research, and ongoing new therapies. To serve as an advocate for the patient and/or living donor and liaison between the transplant center, physician offices, the hospital, and the dialysis facilities. The Pre-Transplant Clinical Coordinator must take kidney donor calls and facilitate in hospital transplant admissions on a rotating 24/7 basis. Serve as back up nurse in transplant clinic and stay updated on other job functions as a resource. Be committed to ongoing personal education in the specialty area.
