Job Summary:
The Care Management Team Lead Certified is a responsible professional nurse employed by Erlanger Medical Center who is responsible for assisting the patient, families, physicians and staff in managing the present health care plan. The incumbents acts as a liaison between care management staff, doctors, and families when difficulties arise with the care plan or discharge. Incumbent may also be asked to represent the department on boards or committees where the Care Management Department has an impact.
This position is also responsible for the coordination of daily assignments for Utilization Managers, Case Managers and Social Workers. They will be the case manager for the Emergency room and in the event of staffing shortages, they will assume the duties of discharge planning or Utilization management as the need arises on the floor. This position monitors and oversees the documentation, of RN Case Managers, Social workers, and Utilization Management. They will serve as an educational resource for other department's staff and physicians regarding discharge planning, care transitions and utilization management. This position reports to the Director of Care Management.
Education:
Required:
Graduate from an accredited school with Associates in Nursing (RN)
Preferred: Bachelor's Degree in Nursing, BSN
Experience:
Required:
Broad clinical experience with a minimum of three years experience and evidence of clinical knowledge.
Preferred:
Experience preferred in both utilization management and case management. Knowledge of managed care practices and familiarity with community resources, regulatory and insurance reimbursement issues preferred.
Position Requirement(s): License/Certification/Registration
Required:
Current licensure to practice nursing in the State of Tennessee (RN). Certified Case Manager.
The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.
