This is not a crisis response role but a high-frequency, low-barrier service aimed at connecting people with ongoing care and resources. The PCE Navigator provides support in community settings, including homes, shelters, and public spaces. Key Responsibilities: Receive referrals and conduct outreach within 24-48 hours. Identify immediate service needs and barriers to accessing care. Assist clients in obtaining necessary resources such as housing, healthcare, and benefits. Facilitate engagement with behavioral health treatment and social services. Provide transportation or accompany clients to appointments as needed. Maintain accurate records of interactions, referrals, and outcomes. Collaborate with local crisis intervention teams and service providers. Participate in community planning and training initiatives. Qualifications: Associate’s degree or certification as a New York Certified Peer Specialist (NYCPS) or Certified Recovery Peer Advocate (CRPA). Three years of experience in social, behavioral, or human services. Two years of experience working in the community without direct supervision. Strong interpersonal and advocacy skills. Ability to work flexible hours, including evenings and weekends. Access to a reliable vehicle with insurance and a valid NYS driver’s license.
