Compensation (Based on experience and qualifications)
Location: 4720 S California Ave
Schedule: Monday Through Friday, Evening or weekend work required.
BHP will be responsible for providing services related to the delivery of mental health care to assigned clients at the designated clinical location both in person and telehealth. BHP is expected to provide .9 FTE clinical sessions and .1 FTE administrative session a week. Clinician will also provide behavioral health counseling to children, adolescents, adults, and families in the surrounding community and provide same day behavior health services to patients who may be in distress and in need of immediate assistance.
Primary Duties and Responsibilities: (The following duties and responsibilities are all essential job functions except for those that begin with the word “May.”)
- Provide comprehensive assessment and diagnosis of behavioral health patients which may include patients with substance use disorders.
- Provide effective treatment planning and assist patients in successfully achieving goals which may include with substance use and medication assisted recovery.
- Evaluate behavioral health crises and apply appropriate interventions.
- Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Provides consultation for school staff regarding mental health issues of clients (as appropriate).
- Conducts group therapy as necessary.
- Participates in the collection of mental health information and student outcomes data to be used for monitoring the overall performance and effectiveness of services provided.
- Maintains regular contact with the parent/guardian of each patient (if appropriate).
- Identify problems related to patient services and make recommendations for improvement.
- Develop contacts with community resources and serve as patient advocate as appropriate.
- Participate in the development and modification of clinic programs as indicated.
- Enhance professional growth and development through participation in education programs, in-service meetings, and continuing education seminars/workshops.
- Represent Esperanza in professional and community organizations assigned.
- Provide group psychoeducation therapy to community organizations or onsite.
- Provide supervision to students or unlicensed behavioral health personnel as assigned by Behavioral Health Manager and/or Associate/Director of Behavioral Health.
- Participate in staff meetings and the quality improvement program.
Requirements
- Master’s Degree in Social Work or related field
- Related Clinic/Hospital experience
- Bilingual, English/Spanish fluency strongly preferred
- Proficiency with Microsoft office suites
- Current Illinois license as Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, preferred
- Clinicians eligible for full licensure for LMFT, LCPC, or LCSW and awaiting examination.
- Function as a team member to assure patients obtain access to quality care.
- CADC or experience providing substance use counseling preferred but not required.
