Essen Health Care is seeking a Primary Care Nurse Practitioner/Behavor Healthcare to join our dynamic team. In this role, you will provide comprehensive healthcare services to patients, focusing on preventive care and chronic disease management. Your contributions will be vital in enhancing the health and well-being of our diverse patient population.
Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Provide primary care services including diagnosis, treatment, and management of acute and chronic conditions.
- Administer injections and perform minor procedures as needed.
- Educate patients on health maintenance and disease prevention strategies.
- Collaborate with multidisciplinary teams to ensure coordinated patient care.
- Document patient interactions accurately in electronic health records.
- Participate in quality improvement initiatives to enhance service delivery.
Requirements
- Master's degree in Nursing with a focus on Family or Adult Primary Care or Behvior Health
- Current Nurse Practitioner license in the state of New York.
- Strong knowledge of internal medicine and geriatrics is essential.
- Experience administering injections and managing acute pain preferred.
- Excellent communication skills with a compassionate approach to patient care.
Rate of Pay: $120k—$140k;
