Job Summary
The Vice President of Population Health is responsible for designing, implementing, and managing population health strategies to improve patient care and reduce health disparities. The Vice President works collaboratively with healthcare providers, community organizations, and payers to develop care models that enhance patient outcomes and optimize resource use. This role involves leading data-driven approaches to monitor, assess, and improve population health initiatives.

- Develop and implement care coordination and case management programs targeting chronic disease management, preventive care, and health promotion.
