Job Description
We are seeking a highly skilled and compassionate Caseworker to join our team of dedicated professionals. As a Caseworker, you will be responsible for providing expert case management services to clients and their families, helping them navigate life's challenges and achieve stability and self-sufficiency.
Responsibilities
- Assess client needs and develop personalized service plans
- Connect clients with community resources and support services
- Monitor client progress and adjust service plans as needed
Requirements
- Bachelor's degree from a regionally accredited or New York State Registered college or university
- Or an Associate's degree and 3 years of experience providing Human Services to the public
- Possession of a valid New York State Driver's license at the time of appointment
