Key Responsibilities:
- Provide case management services to clients and their families, including assessment, planning, and implementation of services.
- Develop and maintain relationships with community resources and partners to ensure access to necessary services and support.
- Collaborate with other professionals, including healthcare providers, social workers, and educators, to ensure comprehensive care and support.
- Document client progress and outcomes, using data to inform practice and improve services.
Possession of a valid New York State driver's license is required for this role, as well as a Bachelor's degree in a related field or an Associate's degree and 3 years of experience providing human services to the public.
