Job Title : Hospice On-Call Registered Nurse (RN) Weekend Full-Time
Location : All of the Hospice Service Area (North Atlanta, Atlanta Metro, South of Atlanta, west of Atlanta Metro)
Salary Range : $88,000 - $95,000 annually, plus mileage reimbursement at $0.50 / mile
Schedule : Every Weekend (Full-Time)

Position Overview :
We are seeking a compassionate and dedicated Hospice On-Call Registered Nurse (RN) to join our new and rapidly growing hospice team. The RN will be responsible for providing on-call hospice care, primarily over weekends, ensuring all care needs are met promptly and professionally, including performing weekend scheduled visits and admissions. The ideal candidate must have at least 2 years of Hospice RN Case Manager (RNCM) experience , be knowledgeable in hospice eligibility , possess strong hospice documentation skills , have timely, effective communication with team members and families, and be able to effectively triage calls in a timely and urgent manner and respond to patients and caregivers within 1 hour in person , as our policy is "If they call, we go." The RN will also collaborate with the Interdisciplinary Team (IDT) and document in HCHB EHR as appropriate. HCHB EHR experience is highly preferred . CHPN certification is preferred . Must possess a kind personality and have integrity to always "do what is right".
Key Responsibilities :
- Provide comprehensive, compassionate, and individualized care to hospice patients and their families during weekend shifts, following a patient-centered care plan.
- Triage incoming calls effectively and urgently, ensuring that the right level of care is provided based on the situation.
- Respond to patient / caregiver needs within 1 hour in person , following the policy of "If they call, we go."
- Administer medications, treatments, and provide symptom management for hospice patients.
- Collaborate with the Interdisciplinary Team (IDT) to ensure holistic and coordinated care for the patient, including physicians, social workers, chaplains, and hospice aides.
- Ensure accurate and detailed documentation of patient care activities, assessments, and changes in condition, adhering to hospice guidelines and regulations.
- Document in HCHB EHR system as required, ensuring proper record-keeping for all care provided.
- Educate patients and their families about end-of-life care, symptom control, and coping strategies.
- Coordinate care for hospice patients and communicate effectively with family members regarding the patient's status.
- Support the emotional needs of patients and families during this critical stage of care.
- Assist with coordinating admissions and discharges as needed, following appropriate procedures.
- Provide weekend scheduled visits , including admissions for new hospice patients.
- Must be knowledgeable in hospice eligibility criteria and ensure proper documentation for eligibility and ongoing care.
- Ensure compliance with hospice policies, as well as local, state, and federal regulations.
- Provide coverage for on-call shifts on weekends, including after-hours and emergency response.
- Maintain timely, strong, and effective communication with families, healthcare providers, and other team members.
- Travel to patient homes as needed, within the assigned service area.
