Job Summary
The Case Manager for the Department of Family Medicine at Quillen College of Medicine is an integral member of our healthcare team, dedicated to delivering exceptional care and support to our clients. Working under the guidance of the Case Manager Supervisor, this role involves conducting comprehensive assessments, crafting personalized care plans, and overseeing the implementation and evaluation of tailored care strategies.

In this position, you will play a pivotal role in enhancing client management and satisfaction by seamlessly integrating case management, utilization review, and goal planning into every phase of client care. Your focus on accountability and excellence will ensure effective coordination of services and the achievement of client goals.

Knowledge, Skills and Abilities

  • Knowledge of the principles of growth and development over the life span.
  • Knowledge of policy, principles, and practice of social work and/or psychology, individual and group behaviors, socioeconomic factors, and their relationship to mental health. Has resources at hand for local, state, or national needs for individuals with substance use and/or mental health concerns.
  • Knowledge of managed healthcare programs, including Tenn Care, eligibility issues, and insurance.
  • Knowledge of guidelines and methods used for applying for disability and for accessing vocational rehabilitation services.
  • Knowledge of systems is involved in the provision of healthcare.
  • Ability to provide excellent customer service and support.
  • Ability to interact professionally with persons from diverse cultural, socioeconomic, educational, racial, ethnic, and professional backgrounds effectively.
  • Ability to work with managers, co-workers, members of the public, and professional groups Ability to organize and prioritize workload and meet deadlines effectively.
  • Ability to consistently demonstrate a positive/proactive customer service attitude.
  • Ability to maintain ethical behaviors exemplary of quality public service and fair
  • Ability to work as an effective team member, function independently, and exercise sound judgment
  • Ability to maintain confidentiality.
  • Ability to work within a regional and state service network and interact with other healthcare
  • Ability to utilize computer programs to prepare reports and assist with grant applications.
  • Ability to organize and maintain an accurate filing system.
  • Ability to work independently with minimal supervision.
  • Working knowledge of Microsoft Office and Electronic Health Records.

Required Qualifications

  • Bachelor’s degree in relevant field and

Or

  • An equivalent combination of education, training, and experience
  • Two (2) years of experience

Preferred Qualifications

  • One year of experience working with addiction and/or mental health diagnoses

Job Type: Full-time

Pay: $21.50 per hour
