Position Overview:
We are seeking a compassionate and highly organized Admissions Coordinator / Social Services Director to join our skilled nursing facility team. This dual-role position is responsible for coordinating all aspects of the admissions process while supporting the emotional and social needs of our residents and families. The ideal candidate will have a heart for the elderly, excellent communication skills, and a collaborative spirit that thrives in a team-oriented environment.
Key Responsibilities:
Admissions Coordination:
- Serve as the primary point of contact for hospital case managers, physicians, families, and insurance providers to coordinate admissions for both skilled therapy patients and long-term care residents.
- Facilitate smooth transitions from hospital to facility by gathering all necessary medical and financial documentation.
- Clearly explain Medicare co-pays, patient liabilities, and Medicaid eligibility requirements to residents and their families.
- Assist with and monitor the Medicaid Pending process, ensuring families are informed and supported throughout.
- Provide thoughtful, informative tours to prospective residents and their families.
Social Services Responsibilities:
- Serve as a supportive resource for both new and current residents and their families.
- Advocate for residents’ emotional, social, and psychological well-being.
- Assist with care plan meetings and ensure resident rights and dignity are maintained.
- Work closely with interdisciplinary team members to resolve concerns and maintain high-quality resident care.
Qualifications:
- Prior experience in admissions, case management, or social services in a healthcare or long-term care setting preferred.
- Strong knowledge of Medicaid eligibility and Medicare billing, including co-pays and patient liabilities.
- Exceptional communication and interpersonal skills, with the ability to explain complex financial and healthcare concepts with clarity and compassion.
- Ability to work collaboratively with team members across departments.
- Warm, welcoming personality with a deep respect for the elderly and their families.
Preferred Skills:
- Familiarity with EPIC Care Link and PointClickCare
- Bilingual a plus
- Degree in Social Work, Human Services, or related field preferred but not required
