Job Summary

Duties
- Conduct comprehensive assessments and evaluations of patient needs in accordance with Medicare guidelines.
- Oversee the completion and accuracy of Minimum Data Set (MDS) assessments for all patients.
- Collaborate with healthcare professionals to develop individualized care plans based on assessment findings.
- Ensure proper documentation in medical records to support clinical decisions and billing processes.
- Facilitate discharge planning by coordinating with various departments and ensuring that all necessary resources are available for patients transitioning out of care.
- Monitor compliance with regulatory requirements related to MDS submissions and clinical documentation improvement initiatives.
- Provide training and support to staff on MDS processes, coding practices, and documentation standards.
- Analyze data related to patient outcomes and quality measures to identify areas for improvement.
Requirements
- Valid Licensed Practical Nurse (LPN) license.
- Proven experience in MDS coordination or similar roles within healthcare settings, particularly in SNF/Long Term Care environments.
- Strong knowledge of Medicare regulations, medical coding practices, and DRG systems.
- Familiarity with clinical documentation improvement strategies and best practices.
- Excellent organizational skills with the ability to manage multiple priorities effectively.
- Strong communication skills for collaboration with interdisciplinary teams and patient education.
- Proficiency in electronic health record (EHR) systems and medical records management.
