Job Summary
The MDS Coordinator plays a crucial role in ensuring compliance with Medicare regulations and enhancing the quality of care provided to residents. This position involves coordinating the assessment process, managing clinical documentation, and collaborating with interdisciplinary teams to optimize resident outcomes. The ideal candidate will possess a strong understanding of medical coding, discharge planning, and clinical documentation improvement, particularly within a SNF/Long Term Care environment.

Duties

  • Conduct comprehensive assessments and evaluations of patient needs in accordance with Medicare guidelines.
  • Oversee the completion and accuracy of Minimum Data Set (MDS) assessments for all patients.
  • Collaborate with healthcare professionals to develop individualized care plans based on assessment findings.
  • Ensure proper documentation in medical records to support clinical decisions and billing processes.
  • Facilitate discharge planning by coordinating with various departments and ensuring that all necessary resources are available for patients transitioning out of care.
  • Monitor compliance with regulatory requirements related to MDS submissions and clinical documentation improvement initiatives.
  • Provide training and support to staff on MDS processes, coding practices, and documentation standards.
  • Analyze data related to patient outcomes and quality measures to identify areas for improvement.

Requirements

  • Valid Licensed Practical Nurse (LPN) license.
  • Proven experience in MDS coordination or similar roles within healthcare settings, particularly in SNF/Long Term Care environments.
  • Strong knowledge of Medicare regulations, medical coding practices, and DRG systems.
  • Familiarity with clinical documentation improvement strategies and best practices.
  • Excellent organizational skills with the ability to manage multiple priorities effectively.
  • Strong communication skills for collaboration with interdisciplinary teams and patient education.
  • Proficiency in electronic health record (EHR) systems and medical records management.
