Job Description : We are seeking a compassionate and skilled Physical Therapist to join our team at our Skilled Nursing Facility in McMinnville, Oregon.

The successful candidate will work directly with patients to develop and implement individualized treatment plans aimed at improving mobility, strength, and overall physical function. Key Responsibilities :
- Perform evaluations and assessments of residents to develop personalized treatment plans.
- Provide direct patient care, utilizing evidence-based interventions to improve physical function.
- Educate patients and their families on exercises, pain management, and mobility strategies.
- Document patient progress and communicate effectively with interdisciplinary teams.
- Collaborate with nursing staff, physicians, and other healthcare professionals to provide comprehensive care.
- Supervise and mentor physical therapy assistants and support staff.
- Adhere to all regulations and standards of care within the Skilled Nursing Facility setting. Qualifications :
- Current, valid Physical Therapist licensure in the state of Oregon.
- Degree from an accredited Physical Therapy program.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Experience in a Skilled Nursing Facility or similar healthcare setting preferred but not required .
