About This Role :
The Behavioral Health Case Manager plays a vital role in providing ongoing support and care coordination for patients with behavioral health needs and their families. Through comprehensive assessment, goal setting, implementation, and evaluation, this position ensures that individual and family needs are met effectively. The primary focus is addressing social determinants of health by facilitating care coordination, follow-up, and fostering patient and family engagement in behavioral health therapy. This role emphasizes enhancing patient outcomes, satisfaction, and continuity of care while promoting quality patient management. With accountability for the planning and coordination of a dedicated caseload, the Behavioral Health Case Manager supports providers and therapists by helping patients achieve their personalized goals. As an integral member of the care team, this position aligns with Evara Health's mission, vision, and commitment to delivering patient-centered, high-quality care.
What You'll Do :
- Provide case management for assigned Evara Health patients.
- Screen and assess patients for mental health, substance abuse, and social determinants of health to create patient-centered care plans.
- Monitor patients in-person or virtually for progress and complications.
- Facilitate referrals to internal and external services, ensuring successful linkages are documented.
- Reinforce clinical visit orders, medication compliance, labs, and referrals through record review.
- Maintain regular contact with patients, update care teams, and document interactions.
- Support patients with active Safety Plans through scheduled check-ins.
- Collaborate with care teams to adjust treatment plans as needed.
- Advocate for patients and their families within the community.
- Track outcomes using a patient registry and complete quarterly summaries.
- Build trusting relationships through education and outreach.
- Deliver culturally competent, solution-focused, and recovery-oriented care.
- Use crisis intervention skills and report risks to supervisors.
- Document all patient interactions in accordance with Evara Health standards.
- Participate in consultations, team meetings, and performance reviews.
- Maintain punctual attendance and strict confidentiality.
- Adhere to all Evara Health, HIPAA, HRSA, and regulatory standards.
- Perform other duties as assigned.
