About the Role :
Under general supervision, the QPCM analyzes performance data, identifies trends, and reports findings to support data-driven decision-making. This role ensures compliance with UDS quality measures and drives continuous quality improvement. Focused on improving health outcomes, the QPCM provides education on HIV prevention (PrEP), care coordination for infectious diseases, and expands PrEP access and adherence. Through collaboration with multidisciplinary teams, the QPCM enhances patient-centered care and overall health impact.
What You'll Do :
- Develop and implement personalized care plans to improve health outcomes.
- Monitor patient progress and ensure adherence to treatment guidelines.
- Collaborate with healthcare providers to address barriers to care.
- Educate high-risk patients on PrEP, nPEP, and prevention strategies.
- Facilitate access to PrEP and follow-up care for at-risk individuals.
- Track and report HIV screening, diagnosis, and prevention efforts.
- Coordinate public education initiatives in collaboration with Marketing.
- Train team members on HIV prevention strategies.
- Ensure timely treatment and support for newly diagnosed HIV patients.
- Assist with referrals and navigation of healthcare systems.
- Address clinical, social, and behavioral health needs.
- Educate patients on disease management, medication adherence, and prevention.
- Provide culturally competent care and advocate for patient needs.
- Support quality initiatives and ensure regulatory compliance.
- Maintain accurate patient records and monitor clinical quality metrics.
- Analyze data, track trends, and present findings to leadership.
