Key Responsibilities
- Assess and prioritize resident needs, developing individualized care plans to promote their physical, mental, and psychosocial well-being
- Collaborate with the interdisciplinary care team to ensure comprehensive and coordinated care
- Monitor residents' progress, documenting medical records and incident reports as needed
- Communicate effectively with residents, families, and the healthcare team
Requirements
- Active Minnesota RN license or ability to obtain upon hire
- Experience in Long Term Care preferred but not required
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
