Social Worker
Position is based at Somerset, NJ main office.

Summary
This position is a member of our Clinical Department and will work directly with our Clinical Director and requires regular visits to our Residential Group Homes and Day Programs throughout the several counties that we service, along with meetings, both virtually and at our main office, in Somerset. The successful candidate will be self-motivated, detail-oriented, and have a strong ability to prioritize under strict timelines and competing priorities.
Social Worker duties include but are not limited to :
- Review individual referral packets and interview potential clients to determine eligibility for Everas services.
- Coordinate the intake process and annual acknowledgments to receive / maintain services from Everas.
- Participate as a member of the client’s Interdisciplinary Team (IDT) to ensure individuals receive correct, comprehensive services and that their goals are appropriate.
- Coordinate services to enable individuals to live successfully in the community ensuring access to community resources, vocational opportunities, etc.
- Ensure individuals’ personal needs are met by securing financial and resource eligibility and that it is maintained i.e. Medicaid, SSDI, SSI, etc.
- Responsible to visit all individuals at their homes and vocational settings to ensure they are receiving the appropriate services and act as a knowledge and training resource to Everas program staff.
- Develop and maintain excellent relationships with Support Coordinators, individuals, their families and guardians to ensure individuals receive comprehensive quality services.
