- Part Time 1st Shift (6a-2p)
- Part Time or Full Time 2nd Shift (2p-10p)
- Part Time & Limited Full Time Available
- Every Other Weekend
- Every Other Holiday

Caregiver / CNA Job Description
Performs resident care activities and related services necessary in caring for the personal needs, safety, and comfort of residents as assigned. Assists in providing a positive physical, social, and psychological environment. Follows all Evergreen policies and procedures.
CAREGIVER / CNA ESSENTIAL FUNCTIONS
Assists with resident/patient care needs and comforts including but not limited to:
- Assists or gives oral hygiene and mouth care.
- Assists with bathing functions and dressing/undressing as necessary.
- Assists with nail care, shaving, and hair care.
- Assists with lifting, turning, positioning, and transporting residents.
- Keeps residents dry (i.e., change clothing, linen, etc.) when he/she becomes soiled or wet.
- Assists resident with bowel or bladder functions.
- Check residents frequently to ensure that their personal care needs are being met.
- Prepares residents for meals (i.e., take to bathroom, wash hands, oral care, take to and from the dining room, etc.)
- Serves food trays. Assists with feeding as needed.
- Performs after-meal care (i.e., clean residents’ hands, face, clothing, brush teeth, take to bathroom, etc.)
- Notes and reports changes in resident eating habits, especially those on a diabetic diet.
- Assists with keeping nursing and resident care areas neat, sanitary, and orderly.
- Appropriately records and charges for supplies.
- Follows work assignments and schedules.
- Complies with laws and regulations applicable to position and complies with the Evergreen Compliance Program.
- Attends and participates in in-service training and other meetings as scheduled and directed.
- Treats all residents, visitors, and staff with courtesy.
- Assists in orientation and training of employees as assigned.
2. Caregiver / CNA Safety
- Knows and follows facility rules.
- Demonstrates proper use of equipment. Reports equipment in need of repairs or maintenance.
- Follows facility smoking policies.
- Reports and documents any incidents or accidents of residents, staff, or visitors to the appropriate facility personnel.
- Performs duties, which may include transportation of residents, as assigned in the Facility Disaster Plan.
- Uses required protective equipment.
- Follows infection control standards, policies, and procedures.
- Reports all hazardous conditions/equipment to the supervisor.
3. Resident Rights for Caregivers and CNAs
- Knows Resident Rights. Helps the residents exercise and/or protect their rights.
- Reports resident complaints to management.
- Maintains confidentiality of resident information.
4. HIPAA rules for Caregivers and CNAs
- Follows and adheres to Evergreen policies and procedures implementing HIPPA requirements for the privacy and security of protected health information.
- Uses and/or discloses only the minimum amount of Protected Health Information necessary to complete assigned tasks (applies only if the position requires access to PHI under Role-Based Access).
- Reports all suspected violations of the company’s HIPAA policies or procedures to the Administrator.
5. Physical & Mental Requirements
- Ability to lift, carry, push, or pull up to 50 pounds unassisted and over 50 pounds with assistance.
- Capacity to stand, walk, bend, kneel, or squat for extended periods.
- Manual dexterity and fine motor skills for handling medical equipment, assisting residents, and performing assistance tasks.
- Ability to assist residents with mobility, including transfers from beds, wheelchairs, or chairs.
- Adequate visual and auditory acuity to monitor residents’ needs and respond to emergency situations.
- Patience and resilience in managing challenging behaviors and situations, including tolerance for occasional verbal or physical aggression from residents with cognitive impairments or other conditions.
- Strong interpersonal skills to provide compassionate and respectful care to residents and communicate effectively with staff and families.
- Ability to handle high-pressure situations, including medical emergencies, calmly and efficiently.
- Flexibility to adapt to varying daily tasks and responsibilities.
- Comfortable working in close proximity to residents and exposure to bodily fluids, odors, and other conditions typical in a healthcare environment.
- Ability to work in a team-oriented setting while independently managing assigned responsibilities.
- Willingness to work rotating shifts, including nights, weekends, and holidays as needed.
*Employees including CNAs and Caregivers must average no less than 30hrs./wk. to be eligible for health insurance benefits.

Pay: $16.00 - $18.00 per hour
