- Conduct case management duties to ensure clients receive appropriate resources and services.

Experience
Candidates should possess experience in the following areas:
- Knowledge of care plans and case management principles.
- Familiarity with HIPAA regulations and compliance standards.
- Experience in hospice care or social work is highly desirable.
